This option is for reoccurring payments or for our one-time payment programs to be broken up into multiple payments. Our Office Manager, Mallory, will contact you to set up reoccurring auto-payments. Our system does NOT save your credit card info after first payment. If you opt out of setting up monthly payments, you will get a monthly text reminder providing a link to pay. Late fee for monthly payments is $5.00

I understand the inherent risks in participating in this athletic program. I hereby release and will not hold ST Sports or any representative responsible (or liable) for any accidents during normal class participation.

I understand that ST Sports staff will do everything possible to protect my child from accident and injury.

I agree that ST Sports may take videos or pictures so that they can keep parents updated on progress or for publicity reasons.
