I understand that Story Time Soccer staff will do everything possible to protect my child from accident and injury.

I agree that Story Time Soccer may take videos or pictures so that they can keep parents updated on progress or for publicity reasons.

I understand that my child(ren) is/are automatically enrolled in the following program unless Story Time Soccer receives a cancellation notice.
